The diet is mainly tsampa, horse tea and beef

Tibetan herdsmen travel five hours to see anorectal diseases

Chen Bangfei and Wu Wanzhen helped the medical unit is the people's hospital in Markang, Aba, Sichuan Province, which is also the preferred hospital for Tibetan herdsmen. On the day of Chen Bangfei's arrival, a Tibetan herdsman in his forties came to the hospital with unspeakable pain. His family lives 100 kilometers away from Malcom City on the Tibetan plateau, not in front of the village, not in the back of the store, the family only relies on raising oyster cattle to increase income, living "the sky as a room, the ground as a bed" days. Due to the local dry climate and lack of drinking water sources, the diet is mainly tsampa, horse tea, beef, fresh fruits and vegetables have become a luxury, these and anorectal disease inevitably involved.

The patient had been suffering from painful prolapse of anorectal masses and bleeding for half a month. He complained that he had had blood in his stool many years ago, but had put it off again and again because of the inconvenience of seeking medical attention. This time he traveled more than five hours to come here, which is really not easy. Chen Bangfei listened carefully to the patient and his family and made a careful examination and assessment of the patient, and together with Wu Wanzhen did a good job of psychological comfort, helped guide the local hospital medical staff to make immediate preoperative preparations, and personally performed surgery for the patient, relieving him of the chronic disease that had plagued him for many years. At the same time, he repeatedly and patiently explained the structure of his diet and the points to note in his life routine. The doctors at the local hospital also did their part and planned to organize regular follow-up visits to the homes of distant Tibetan herdsmen in the countryside.

Do schizophrenic patients often drool?

Or problems with medication

In the psychiatric ward of the People's Hospital in Markham, Wu Wanzhen is rechecking Ms. Yu (a pseudonym).

Ms. Yu looks young and pretty, dresses stylishly, and behaves appropriately. However, since two years ago, her state has been different. When it was time to go to bed at night, her brain became very excited and kept thinking about things, trying to calm down and relax, but it was useless. When she was alone, she often heard people talking next to her, which made her uneasy and even developed a fearful mind. "I always feel that someone is spying on me, trying to steal my things, and trying to harm me, so I have no choice but to call the police." Ms. Yu's temper was getting worse and worse, knowing that she was wrong to do so, but she couldn't figure out why it was so. Such a state has seriously affected her work and life.

Six months ago, she came to Malcolm City People's Hospital for medical treatment, and after a preliminary diagnosis, Ms. Yu was suffering from "schizophrenia. Wu Wanzhen said, "At that time, the doctor prescribed her 1,000 mg of clozapine, which led to the phenomenon of constant salivation (drooling). The incidence of adverse reactions to clozapine salivation is 40% to 82%, and 31% to 54% of patients experience social humiliation due to salivation, which seriously affects patients' social functioning and compliance with medication. And patients are prone to aspiration infectious pneumonia and even the risk of asphyxiation due to regurgitation of saliva from the respiratory tract during sleep." Under Wu Wanzhen's guidance, local doctors immediately made symptomatic adjustments to the patient's medication.

During the aid to Sichuan, Wu Wanzhen carefully inquired about the condition, medication reaction and psychological status of patients in the hospital, checked the incoming and outgoing registers, inpatient medical records, patient morbidity reports, etc., provided training on whether patients need combined medication, evaluation of the effect after medication, standardized writing of medical records, and management of severe mental disorders, and offered valuable advice and suggestions. "We hope to improve the diagnosis and treatment level of primary care doctors through such activities, so that more patients can benefit." Wu Wanzhen said.

No more menstruation?

The hidden danger of premature ovarian failure should not be ignored

Ram (a pseudonym) is 39 years old, but has not had a period for several months, and for two years before that, periods were messy. When she found Zhou Jinhong at a charity clinic at the Aba People's Hospital, she learned more about Ram's menstrual status, but Ram said that she didn't care if her periods were gone, but recently her body has been suffering from hot flashes and insomnia. Zhou Jinhong initially diagnosed Ram with early-onset ovarian insufficiency (premature ovarian failure). This gynecological disorder refers to the phenomenon of amenorrhea before the age of 40 due to ovarian failure. Its dangers include causing premature osteoporosis in women, increased risk of vascular and neurocognitive diseases, and a general decrease in average life expectancy, among other conditions. Women of all relevant ages need to be aware of this, and in the case of Ram, hormone replacement therapy is required.

To effectively fulfill the responsibility of helping to help improve the help unit to improve the level of service and service quality, Zhou Jinhong also through access to cases, view cases, and departmental doctors to carry out exchanges and interactions, to grasp the commonality and characteristics of regional diseases, and put forward the corresponding treatment plan and so on. An anemic patient whose hematocrit dropped to 6g/dl (the normal value of hematocrit for adult women is 11-15g/dl) was waiting to undergo a myomectomy. Zhou Jinhong advised the patient not to rush into surgery, but to undergo further vaginal ultrasound and other related examinations based on the original abdominal ultrasound to clearly diagnose the anemia as caused by fibroids, and then perform surgery to help the patient's health problems in a targeted manner. He gave full play to his personal technical expertise, and his rigorous work attitude was well received by patients and local medical staff.

A 1.5-year-old Tibetan child

Fever and cough for 3 months found a savior

Hearing that a pediatrician from Zhejiang had come all the way here, Zhuoma (a pseudonym), who had received the news, rushed to Jiuzhaigou County People's Hospital with her 1.5-year-old child and specifically sought professional advice from Shen Daojiang. The child had been coughing and feverish for 3 months, and had been diagnosed with pneumonia and hospitalized 3 times. Gina's (pseudonym) child was just over 2 months old and had a violent cough before being admitted to the hospital and showed symptoms of shortness of breath and difficulty in breathing. Shen Daojiang gave professional guidance and treatment advice.

In addition to consulting with difficult children, he also conducts room visits, gives lectures (on the topic of "clinical diagnosis of pediatric convulsions and emergency treatment procedures") and conducts "neonatal resuscitation" first aid drills ....... He was busy non-stop. Back in Hangzhou, he was still concerned about those innocent children and their condition, many local children with difficult conditions were recommended to be transferred to Mianyang (6-7 hours drive) and Chengdu (more than 10 hours drive), which is very unfavorable to the development of the disease. "The doctor is a parent, guarding the health of the child, I hope to have the opportunity to go over often to see." Shen Daojiang said.
