“West Lake Mode” is an innovative template forged by the Hospital Integrated with Traditional Chinese Medicine and Western Medicine in West Lake District of our hospital integrating with hospital-community-family. Carrying out “ward area medical entrusted management, and key support for outpatient”, establish “Grade A Class III Hospital Ward”, forge an organic combination integrated with community rehabilitation, medical and endowment integration and tranquil nursing, and form the hierarchical diagnosis and treatment mode of “first diagnosis at fundamental-level, two-way transfer and upward and downward linkage”.
To comprehensively improve the medical quality of custody ward, further improve the overall level of medical service and service ability, according to the principle of homogenized management, the characteristics of the Hospital of Integrated Traditional Chinese and Western Medicine, the medical department formulated personalized quality assessment indicators, regularly to the ward, the inspection results as the department target assessment evaluation basis, also for the west lake model for further exploration and research.
The assessment group led by Director Chen Xinyu came to the Hospital Integrated with Traditional Chinese Medicine and Western Medicine for routine inspection and guidance at 1:30 on the afternoon of April 30, 2019. The assessment group experts strictly referred to the assessment rules, through investigation, accessed to medical records, visited patients, field questions, case tracking, around the hospital functional tasks, ward management, medical services, patient safety, medical quality and safety management and continuous improvement, physician basic skills, critical value management, condition assessment, emergency plan and other aspects of supervision inspection, and discovered the problems and deficiencies on site and offered their guidance.
During the period, Director Chen Xinyu also had a detailed understanding of the implementation of the "discharge preparation plan" in the ward of integrated traditional Chinese and western medicine, communicated with the department director and frontline clinical medical staff, and put forward suggestions on the specific embodiment of discharge preparation service in the medical records: the ward medical staff are required to work with the Information Department of Zhejiang Hospital to embed the form of the discharge preparation plan into the information system, and to realize the interconnection of the evaluation information between the medical staff. From the first day of admission, a personalized discharge preparation plan was formulated for elderly patients, and throughout the hospitalization process to make full preparations for the smooth return of elderly patients to their families. The evaluation and management mode also hopes to be comprehensively promoted and implemented in each ward of Zhejiang Hospital, and put the concept of continuous medical treatment required by JCI evaluation runs through every corner of the hospital.
