On May 20, Zhejiang Hospital and Haiyan County Health and Health Bureau held a signing ceremony for the cooperation between the National Comprehensive Pilot Zone for Grassroots Health and Health and the National Key R&D Program "Health Management Integrated Service Application Demonstration". The "Demonstration of Health Management Integrated Service Application" is a national key R&D project led by Southern Hospital of Southern Medical University, with the participation of 21 units including National CDC Chronic Disease Center, National Cancer Center and National Geriatric Medical Center. At present, the project has entered the implementation stage. The main objective of the project is to build a life-cycle health assessment and monitoring system and a health management and service system for the whole population, and to build an "active health-centered" health management service model. At the signing ceremony, Lin Jian introduced that during the 14th Five-Year Plan period, China has transitioned from light aging to moderate aging, and middle-aged and elderly people over 60 have reached 264 million, one third of whom are prone to high incidence of geriatric diseases. Therefore, it is especially important to reduce the incidence of diseases through active intervention during the window period of middle-aged and elderly people to achieve "all-round and whole-cycle protection of people's health". The pilot project in Haiyan County will be oriented to active health, with dynamic identification of health imbalance, health risk assessment and autonomous health management as the main direction, focusing on breaking through the difficulties and bottlenecks such as quantitative stratification of human health status, continuous dynamic collection of health information and integration and analysis of health data, leading to the construction of a new health perception, identification, intervention and management technology system, and developing a technology elderly service standard and evaluation system suitable for China's national conditions. It also provides active support for promoting the transformation and upgrading of health security, building an integrated elderly service system of pension, rehabilitation, nursing and medical care, and accelerating the cultivation of new health industries.

Lu Haifeng said that this cooperation is an important initiative to do practical things for the masses. Haiyan will start from the "pain points" of primary health care services, align with the national requirements for high-quality development of public hospitals, improve the hospital's specialty service capacity and the capacity of grassroots community public health services according to the actual situation in Haiyan, and provide Haiyan citizens with We will provide high-quality, all-round, and full-cycle health services for Haiyan citizens, enhance the practicality of the demonstration area construction, and build a new model of active health that can truly reflect Haiyan's characteristics.

     Promote the construction of health management service model for the elderly

Based on the requirements of the current community public health service standards, Zhejiang Hospital will help Haiyan to establish a geriatric function assessment and health risk assessment and early warning mechanism, study the construction and application of the geriatric function index model, explore the establishment of a multidimensional and stratified health management model for the elderly, integrate multiple health service resources, establish a geriatric health service management information platform, and form an overall geriatric health management service plan.

Using the existing public health service system and integrating multiple resources such as administration, medical care and elderly care, Zhejiang Hospital will innovate informatization management means to carry out the pilot work of elderly health management in a reasonable and orderly manner, and continuously upgrade and optimize the program according to the work carried out.

     Health data integration leads technical cooperation

According to the progress of the pilot project, Zhejiang Hospital will conduct statistics, analysis, summarization and conclusion of the data to form research and construction results with high level, and at the same time improve the service capacity and management level of grass-roots public health service institutions and the technical level of public health service personnel in grass-roots medical institutions through various ways such as technology sharing, education and training, collaborative cooperation and joint development.

After the signing ceremony, Huang Xiong'ang provided special training to hospitals and health centers at all levels in Haiyan County, explaining in detail the specific requirements and steps of physical fitness screening for the elderly. Zhejiang Hospital will also continue to explore a healthy elderly system suitable for China's national conditions through the implementation and implementation of the project in Haiyan, and build up an integrated elderly service system of elderly care, rehabilitation, nursing and medical care, so as to contribute to the continuous improvement of people's health and welfare.
