In order to implement the Opinions on Promoting the High-quality Development of Public Hospitals issued by the General Office of the State Council, the final of the "Seventh Season Action National Hospital Challenge Competition" under the guidance of the Medical Administration of the National Health Commission recently concluded in Qingdao. There are ten themes in the competition, with 2,058 participating projects nationwide. 《Boost maximum one-go, and establish an information-based special medical record filing system for anesthetic drugs》
Narcotic drugs and the first type of psychoactive drugs are drugs under special management in accordance with laws and regulations in China. We should not only ensure the rational clinical use needs, but also strictly prevent the inflow of illegal channels. In order to strengthen the management of the whole process of anesthetic drugs, medical institutions need to improve the information management level of anesthetic drugs. This case focuses on solving the difficulties of effective control of the use of narcotic drugs in outpatients, exploring the construction of a special medical record filing system for patients with cancer pain and patients with chronic moderate and severe non-cancer pain with long-term use of narcotic drugs and class I psychotropic drugs in outpatients, integrate outpatient office workstation, doctor workstation, pharmacy workstation and rational drug use system to seamlessly connect basic patient information and prescription information. The process is simplified, the patient treatment experience is improved, and the work efficiency of medical staff is improved, narcotic drugs are prescribed regularly, and narcotic drugs are used reasonably. To achieve the goal of information, simplification, fine and effective control.
The medical record filing system is jointly constructed by the outpatient department, information center and pharmacy department under the coordination of the medical department. To serve patients as the guidance, with rational drug use as the main line, multi-department cooperation and innovation, to promote the high-quality development of the hospital. The participating projects were strongly guided by the Quality Management Office.
The project also won the second prize in the second "West Lake Cup" Pharmaceutical Innovation Competition in 2022, jointly sponsored by clinical Pharmacy Branch of Zhejiang Pharmaceutical Association, Zhejiang Pharmaceutical Association Hospital Pharmacy Special Committee of Zhejiang Pharmaceutical Association and Zhejiang Pharmaceutical Quality Control Center.

《Improvement of HCC patient safety management in ICU by HFMEA model》
Patients in the intensive care unit (ICU) are in critical condition and mostly experience anxiety and delusions. ICU medical staff usually use physical constraints to prevent unplanned extubation, falling, or bed falling to ensure the smooth progress of medical care. Improper physical constraints will cause skin damage, nerve function damage or heart function damage, and will also lead to depression, anxiety, irritability, delusion and other bad emotions, and even post-traumatic stress disorder, such as long-term psychological injury.
The HFMEA management mode adopted in this project is applied to clinical practice, with classified intervention according to the role involved in the constraint process, training for nurses, and using theoretical training and scenario simulation methods to make nurses feel for themselves; Using information technology to embed all kinds of constraints into the system, so as to intelligently judge the necessity of constraints and regularly remind the implementation of nursing records;Quality control of the restraint implementation process; adopting the relaxation technique of environmental management and physical exercise, to make the patient restraint rate and restraint time decrease significantly, and improve the quality of nursing service.
The use of physical constraints in ICU patients involves many issues such as patient autonomy, dignity and freedom. The HFMEA management model can achieve the safe management of physically constrained patients. The clinical application of this project is beneficial to maximize the reduction of physical constraints, thus improving the level of humanistic care in ICU medical care.
